Beta-Blockers are often withheld from patients with obstructive airways disease, especially those with reversible airways disease due to fear of inducing bronchospasm. We report our single center experience of cautiously treating such patients who have concomitant chronic heart failure (CHF). The use of cardioselective beta-blockers under caution and specialist supervision may be tolerable in many CHF patients with obstructive airways disease, resulting in clinical improvement rather than detriment. A retrospective case notes analysis was performed on CHF outpatients who had obstructive airways disease and been treated with beta-blockers. A total of 43 patients were identified, with an average ejection fraction of 31.8%; 18 of these patients had fixed obstructive airways disease, 15 patients had reversible obstructive airways disease, 10 patients had a label of obstructive airways disease (but no supporting evidence for the diagnosis in the hospital notes). In all 3 groups, beta-blockers had been initiated and maintained without any respiratory event over a median continuous exposure time of 135 days. Limitation of the dose was documented in only 2 patients because of worsening shortness of breath. New York Heart Association (NYHA) class significantly improved for the group with the use of these agents (p = 0.003). A cautious approach (under specialist supervision) to beta-blocker use in patients with heart failure and airways disease can result in successful treatment. The implications of withholding these agents may have more serious consequences than their administration.

Beta-blockers are indicated and have been used successfully for more than 50 years particularly in the treatment of stable coronary artery disease, acute coronary syndrome, myocardial infarction, arterial hypertension, heart failure, supraventricular and ventricular arrhythmias (including atrial fibrillation) and phaeochromocytoma, and in most of these diseases they were shown to be effective in order to reduce morbidity and/or mortality. The „classical“ beta-blockers currently used in cardiovascular diseases showing beta1-selectivity and clinically effective beta-blockade without specific additional effects are Metoprolol, Bisoprolol, Atenolol and Esmolol In addition to their blockade of adrenergic beta-receptors, some of them show additional effects such as Propranolol (inhibition of the conversion of thyroxine to triiodothyronine and herewith particular effectivity in hyperthyrosis; in addition also indicated in portal hypertension, essential tremor and migraine), Sotalol (Class III antiarrhythmic effects and herewith particularly effective in atrial fibrillation, atrial flutter, AVNRTs and ventricular arrhythmias), Carvedilol (blockade of alpha1-receptors and herewith particularly effective in arterial hypertension and heart failure), Nebivolol (NO-derived vasodilation and herewith particularly effective in hypertension and heart failure), Labetalol (blockade of alpha1-receptors and herewith particularly effective in hypertensive emergency, phaeochromocytoma as well as in pregnancy-induced hypertension) and Propafenone (additional antiarrhythmic Class Ic effect and herewith particularly effective in pharmacological cardioversion of recent-onset atrial fibrillation as well as in ventricular arrhythmias) To date, beta-blockers should be used consequently according to their indications. However, one should always keep in mind particularly their side effects as well as the doses recommended by recent guidelines.

Beta blockers play an essential role in the treatment of cardiovascular diseases, but also various other endocrinological, gastroenterological, ophthalmological and neurological disorders. The most important effects of beta blockers are a reduction in myocardial oxygen consumption and inhibition of renin secretion. Beta blockers are divided into three generations according to their selectivity - non-selective, cardioselective and vasodilating beta blockers. Beta blockers and obstructive pulmonary diseases. Patients with obstructive pulmonary diseases are significantly more likely to develop cardiovascular diseases compared to general population, largely due to common risk factors such as smoking, systemic inflammation, age, and genetic predisposition.
